Mono 2-conductor 1/4" open frame jack with tip shunt.
Includes nut (3/8"-32 threading) and washer.
Tip Shunt = Extra lug/circuit/switch that is normally closed (connected to hot lug). It opens/disconnects when something is plugged in.

Industry Standard
This is the input jack that all others are measured against. It's sturdy as can be, will last years, and not prone to oxidation. Beware though, it does not come with a toothed washer - that needs to be purchased separately (NextGen product SKU NG-GPS-B000NSDS6C). Without the washer, it won't make solid chassis ground contact, and will want to spin.

Quality and delivery
I can always count on these guys for having what I need and if not in stock finding product. This jack is used on Fender amps particularly the Blues Junior.

Good jacks... but one warning
As others have commented, the Switchcraft tip shunt jacks are quality and my preferred choice for amp builds. However, occasionally the ground tab on a new jack will bounce back against the hot (tip) tab, after the cable is plugged in, causing a short in the signal. It's an easy fix -- just bend the ground tab outward gently so it clears the other tab. But if you ever find an input or output of your amp shorts out intermittently, this may be the problem.
